npj Digital Medicine, Published online: 12 October 2026; doi:10.1038/s41746-026-03351-5 Ambient AI has spread through clinical settings faster than curricula can adapt, shifting trainee documentation from authoring to editing notes. Authoring historically honed clinical reasoning for trainees, yet medical education is not prepared to develop the editing task now replacing it. We define this task as a teachable competency with four domains - error detection, clinical reconciliation, independent generation, and trust calibration - and describe how simulation can teach them.
